Lifetime Services

As the parent or caregiver of a loved one with a developmental disability or severe and persistent mental illness, you have probably worried about future care for your child or spouse. Through our Lifetime Services program, Jewish Family Services will help you prepare for the future by assisting you to make housing, medical, social, and financial decisions for your loved one’s continued care throughout their lifetime.

Issues

Issues you face may include, but are not limited to, coordinating medical and dental appointments, counseling or therapy sessions, assistance with household chores, money management, social outings, religious participation of any denomination, power of attorney, housing, transportation, and advocacy activities.*

Assessment

A JFS Clinical Social Worker will complete a formal assessment with your family. This confidential assessment includes an extensive personal history, discussion of developmental, physical and emotional disabilities, family traditions, and discussion regarding expected future outcomes.

Planning

A customized care plan is created which include arrangements for Jewish Family Services to provide, coordinate, and monitor the agreed upon services. The plan can be initiated as a whole or in part at any time, or be scheduled to begin after a parent or caregiver's death.  Fees for services are established through a trust which is established by the family.  The assessed fees are commensurate with the services necessary to maintain the healthiest lifestyle for your loved one.

Because of our professionalism, commitment, and familiarity with community resources, Jewish Family Services has been recognized as an outstanding provider of case management to individuals with exceptional needs since 1984.

** It is important to note that this program does not interfere with ones' entitlement to governmental program/subsidies.
