Lifetime Services

As the parent or caregiver of a loved one with developmental disabilities or severe and persistent illness, you have probably worried about future care for your child or spouse. Through our Lifetime Services program, Jewish Family Services will help you prepare for the future by assisting with making housing, medical, social, and financial decisions for this individual's continued care throughout their lifespan.

Issues

These issues may include, but are not limited to, coordinating medical and dental checks, counseling or therapy sessions, assistance with household chores, money management, social outings, religious outings of any denomination and advocacy activities.**

Assessment

A Clinical Social Worker will complete a formal assessment with your family, which includes personal history, physical and emotional disabilities, family traditions, and discussion regarding expected future outcomes.

Planning

A customized care plan is created with arrangements for Jewish Family Services to provide, coordinate and monitor the agreed upon services. The plan can be initiated as a whole or in part at any time, or be scheduled to begin after a caregiver's death. Payment is established through a trust, with fees being adjusted as necessary to maintain an equitable level relative to your loved one's needs.

Lifetime Services can ensure the continued safety and security of your loved one based on your guidance and predetermined wishes.

** It is important to note that this program does not interfere with ones' entitlement to governmental program/subsidies.
