Family CareFamily Care is a division within the Department of Older Adult Services at Jewish Family Services. It is funded by the Milwaukee County Department on Aging. Available Services
The Family Care department serves frail, low income, older adults who might otherwise be forced to move to a nursing home. We provide in-home services such as personal and supportive home care, transportation and home delivered meals. These services make it possible for older adults to continue living in their own homes — and receive the support they need — at a fraction of the cost of a nursing home. Our ClientsIn order to qualify for Family Care, you must be at least 60 years of age and have been identified as having a long-term care need. Since it’s inception seven years ago, the JFS Family Care department has served more than 600 older adults. At any given moment, our staff of 12 full time case managers, 4 part-time case managers, 6 full-time registered nurses, and 2 part-time registered nurses and 1 full-time case aide are working with an average of 545 seniors. Our StaffOur highly trained and professional staff is what makes the JFS Family Care program so exceptional. Nurses play an active role in ensuring a client’s well being, from assisting with coordinating doctor’s appointments, medication management, working with members on range of motion exercises and conducting diabetic training, as well as other health education. The case manager is responsible for overseeing the services within the home and is always willing to lend an ear when the client is lonely and needs someone to listen. Oftentimes, the JFS case manager is the only family an older adult has, and the caring way we comfort and provide services can help ease their current situation.
