LinkAges

JFS offers a comprehensive care management program designed to meet the needs of older adults and their families. These needs may include personal independence, health, social connectedness, nutrition and psychological well being.

To help identify the right level of assistance one needs, a JFS care manager will conduct an assessment and then discuss the options, resources and alternatives available.

Available Services

  • Needs evaluation
  • Preparation of individualized care plans to optimize health and well being
  • Ongoing monitoring and evaluation for continuity of care
  • 24 hour coverage, 7 days a week
  • Liaison for families at a distance
  • Medical management
  • Assistance with bill paying
  • Information and referrals
  • Counseling
  • Coordination of services and resources, including referrals to high quality caregivers, homemakers, home health care, adult day care, and senior residential options

Evaluations are held wherever you are (the hospital, your home, a nursing home). For some families, all that is needed is information and direction. Other situations require on-going contact. JFS LinkAges staff is here to provide whatever is needed.

Care Management

The JFS LinkAges program helps older adults and their families deal with the issues of aging. The LinkAges care management team provides a single source of support which emphasizes:

  • Compassion
  • Quality
  • Affordability
  • Jewish values and tradition

Affordable services are available to individuals of all faiths.

Community Outreach

Older Adult Services offers outreach programs for caregiver support. The Lunch and Learn Wellness program is available to businesses, civic organizations, and church and synagogue groups located in the greater Milwaukee community. The sessions address topics such as: what is normal aging; how to navigate the social service system; how to broach difficult subjects, such as end of life wishes, finances, driving and moving; what community resources are available; and why caregivers should care for themselves. This program can be adapted to an individual group’s needs. JFS also offers additional programming for older adults and their caregivers including, special program events open to the community.
